Causes Of Varicose Veins
If you look at a normal leg, you will see that the venous return is made up of two main circulations. The one deep inside the leg is the femoral vein. The largest superficial vein is the Larger Saphenous Vein. These two are connected together by smaller veins that run through the thickness of the leg called perforators. There are many perforators per leg. Around a few hundred is not uncommon.
Normally blood flows from the surface, into the perforators and then into the deep circulation. There are little valves present in all these vessels that usually make sure that the blood flows only in one direction i.e. from the surface into the deep veins. However this is against a pressure inside your leg when you stand, and without valves the line of least resistance would be from deep veins to the surface veins.
Each time you contract a leg muscle, or an artery pulsates, the venous blood is ‘milked’ back up the leg to the groin and then onto the heart. If however the valves are not working properly as in the case of varicose veins, nothing stops the backflow of blood down the leg. And with the blood volume, flow and pressure pushing back to the surface, the superficial veins become thickened, dilated and stretched. That is what you see as a rope of varicose veins.
Why don’t the valves work? There are several reasons for that. The first one is genetic defects and you are born that way. This especially is the case in persons who present early in life, while still young, with varicose veins. They have numerous recurrences of varicose veins throughout their life and present for many repeat operations and procedures. There is no technology that works whereby we can implant valves, although this has been extensively tried. The second is that the valves are damaged by trauma. A rugby, hockey or cricket ball, for example, to the leg, can often in the long tern leave a varicose vein behind when it heals.
The third and most common has to do with the strength of the vein walls. Each vein is a cylinder with a wall that consists of numerous layers. One of these layers contains collagen fibres of different types. These fibres are kept in their correct shape by chemical bonds. Your DNA in your cells codes for these chemical bonds and this is where the problem lies. Your DNA is producing the wrong number and quality of chemical bonds. This makes the collagen more lax and elastic, than supportive. When the vein wall is too lax, it pulls apart the valves and now the blood can follow the line of least resistance, namely flow back to the surface. The veins enlarge and thicken and are visible as the green/blue ropes below the surface i.e. varicose veins. It is from the varicose veins that so often little red spider veins grow. It seems as if the little red spider veins are trying to get rid of the true underlying varicose veins.
Spider veins are similar to varicose veins in that they develop in the same way as varicose veins. They are however much smaller than varicose veins and they don’t cause the bulging and swelling that varicose veins do. They are closer to the surface of the skin and will be red or blue in color. They look some like the branches of a tree or a spider web. Spider veins vary in size from small patches to large surfaces and occur usually on the legs or face.
